Besides ones own sanity being at risk with every page turned, the most obvious environmental effects would be a earthquake followed by a storm of vengeance. Earthquakes aren't really possible in Sigil, due to it being a ring off the ground, so that sheer dumb luck saved many lives.

The gate opening and demons coming to kill you were out, but ones soul being captured in a gem and buried underneath the ground or straight up disintegration was still on the table.

...No, she was fostering her own sect, of course. It seemed that Akh had too little information to discern the who if it.

She tried a different approach in re-reading the source - any manifestation of the costs exacted by the Codex, environmental effects of its presence and use beyond the effects of its spells.

The list of portals would be found in the Hall of Records, and it would also not include all unknown variables. The only publicly known one is the one in Arborea, though that would require the caster to be a Sensate, since it's inside of the Civil Festhall.

If Akh had to speculate, it would mean that whoever had to book probably would be inclined to set up their base of operations outside of Sigil and use established portals to hop there and back to affect matters in the Cage. Probably the Codex offered way more than supreme planeswalking... She had a thought though.

Unfortunately, beyond the earthquakes, storms, fiendish invasions, imprisonment, and straight up death from being unable to handle its power, it also grants some powerful spells to those willing to put in the work to read it, including plane shift, elemental swarm and a gate spell. If you could handle taxing effect on your mind, the owner of the could become a powerful Planeswalker.

Unfortunately, as you stop and think about for a few moments think, of the spells spells taught within these pages that are recorded in your book, the majority of them are pretty useless within Sigil itself, as the Lady prevents summoning spells beyond the beings within the city itself, not to mention the spells Plane Shift and Gate do not work inside of the CIty of Doors.

Did she have any way to trace those misfortunes until the bearer of the Codex had already been abandoned? She analysed the records once again, looking for some harbingers of the book's use that could help her stop being always a few steps behind.

Unfortunately, that could only be determined by finding the book itself. A rather large needle in a a large planar metropolises. Still, knowing the telltale signs of misfortune upon knowledge learnt would be the best way to figure it out.

Except who the Codex had chosen this time. So Akh kept sitting there, staring forth, wondering how this could be determined.
